Answer
Tetanus vaccine is an inactivated toxin (toxoid) made by growing the bacteria in a liquid medium and purifying and inactivating the toxin. It involves activation of type II Immune response
It is administer once the quickening felt and can be repeated 2-3 months after first injection in primid women as a booster injection. (Usually 5th and 7th months of pregnancy)
For the first time injection, it will stimulate the production of IgM and activation of memory B cell. This IgM could not cross the placenta, hence it action mainly for the protection of the mother.
During the second @ booster dose, it will stimulate the production of IgG. This antibody can cross the placenta and it can give protection for the baby.
Examples of importance of ATT is illustrated in this case.
For woman who undergoes caesarean section, they are always at risk of developing the tetanus from contaminated surgical instrument. ATT are meant to prepare the mother for this event.
As for the baby, the most common source of infection is during the cutting of umbilical cord especially with inadequate sterility of scissors.
Notes: It is different from Anti tetanus human immunoglobulin which are preparation containing IgG immunoglobulin derived from plasma of donors sensitized to tetanus toxoid. It acts by The IgG antibodies acts to neutralize the free circulating exotoxisn of clostridium tetani and prevent its fixation in tissue and its consequences.

salam dik. jzkk. hm, akak wat check up kat private hospital. G1P0, POA : 27, EDD : 17 June 2011. Unfortunately the Dr never mention about ATT. should i get it now?

ReplyDeletewsalam wbt.. ATT patut diberi kepada ibu mengandung terutama yg mengandung kali pertama..first dose adalah apibla rasa bayi mula menendang.. dan second dose boleh diambil 2-3 bulan kemudian

ReplyDeletepuan.. polisi adalah utk memberikan ATT seawal mungkin kepada ibu mengandung dan juga booster dose kerana berdasarkan teori, paras antibodi dalam bayi akan meningkat dari bulan keempat dan akan mencukupi apabila sampai waktu untuk melahirkan bayi.
ReplyDeletekeduanya, jarak di antara dos pertama dan booster juga memainkan peranan penting. Jarak yang sesuai 2-3 bulan memastikan pemindahan antibodi dari ibu ke bayi berlaku dengan effektif
ada masanya ibu mengandung lewat mendapat suntikan pertama mengakibatkan booster dose disuntik sebelum bersalin dan ini akan mengganggu proses pemindahan antibodi dari ibu ke bayi.. walaubagaimanapun, ia adalah lebih baik dari tidak beri langsung dna pemberian ATT juga akan memberi perlindungan utk bayi pada kandungan akan datang
dalam kes puan, saya galakkan puan berjumpa doktor yang merawat untuk berbincang tentang isu ini memandangkan kapasiti saya sebagai pelajar perubatan tidak membolehkan saya memberi kata putus dalam merawat pesakit.. hanya sekadar boleh memberi maklumat dan informarsi..
pendapat peribadi saya, saya cadangkan puan mengambil suntikan tersebut setelah berbincnag dengan doktor yang merawat..

ReplyDeleteSalam Dr. Naim. Why is ATT administered once quickening felt? Any particular reason behind it? And my 2nd q, does a lady still need ATT for her 2nd pregnancy if she already got it during her previous pregnancy?
ReplyDeleteMedications aren’t good during pregnancy as most are contraindicated as the use of drugs may interfere with fetal health.
