This tip is for patient come to Ophthalmology ward, usually with the chief complaint of blurring of vision, cataract, glaucoma and mentioned the history of Diabetes Mellitus. But it is also useful in other medical disciplines

1) Since when?

2) Who diagnosed the DM

3) Follow up for DM in which hospital?

4) Type of medication

5) Compliance to drugs or not?

6) Any history of admission to wards d/t DM complication (Diabetic foot, peripheral neuropathy, hyperglycemia, hypoglycemia, diabetic keto acidosis.)

7) Complication of diabetes

8) On diabetic diet or not.
