Dead after ECT procedure

A 20 years old Malay male with bipolar affective disorder in manic phase was scheduled for Electroconvulsive therapy. Physical examination reveals no abnormality. However he died after the procedure.

A 22 Year old chinese man was brought to A&E department by the police after he showed aggressive behavior towards family member and decrease sleep for three days duration.

During the interview session, he was very irritable and burst into anger whenever the interviewer oppose to what he said.

His speech is pressured and he jumps from another topic to another topic.

He also claims that he is a genius and have power to built nuclear weapon.

SEQ Mental status examination 1

A 34 years old Malay male which was recently diagnosed with mental illness have history of frequently visiting prostitute for sexual activity. A psychiatrist was come to pyschiatry ward to evaluate the patient. Below is some of the conversation between the doctor and patient.


Dr: I was told that you frequestly visit the prostitude, am I correct?

Pt: Yes, I am.

Dr: So, what do you think about it? Is it ok to do that?

Pt: Yes! It’s ok to do that.

Dr: So, you don’t think that it’s wrong.

Pt: Yes.

Dr: What about your wife?

Pt: Well, I’m single

Dr: What did your religion said regarding this thing?

Pt: Well, I did not pray and not really practising my religion.

Dr: Do you aware the dangerous of your activity with prostitude?

Pt: Do you mean HIV or any other sexual transmitted disease?

Dr: Yes.

Pt: Well, I use condom.

mood disorder that gradually relieve on evening

A 40 Years old woman who has lost interest in her work and social life and lack of energy, motivation and appetite for two weeks of duration comes to your clinic. She tells you that although she feels hopeless and helpless most of the time, she seems to feel a bit better in the evening than in the morning [taken and modified from Page 65, Pathology Recall, Anikar Chhabra MD, Lorne H. Blackbourne MD, Lippincott Williams&Wilkins, 2002]

change of mood in 20 years old female students

A 20 years old female college student which previously quiet presented with history of being excessive talkative and active for two weeks of duration. She always gives mini lectures to her friends and become extrovert. Currently she enjoys shopping and spent RM250 for unnecessary item. She starts to wear bright clothes. During your interview, she said that she may become a lecturer as she is very cleaver. She mention that she sleeps for 1-2 hours per night but still very active at days for busying making plans on how to improve the ministry of education. During the interview, she mentioned that she was left by her boyfriend two years ago after giving her virginity and admits that she was depressed with this condition and cries a lot. Sometimes she thought about taking her own life
